Certificate, business/industry license, registration, or permit
Permits and inspection reports from the government (including those for environmental compliance, fire safety, sanitation, and structural safety)
A summary of the operator's certificate and a catalog of inspection and registration certificates for specialized equipment (e.g., forklift, elevator, generator, boiler, pressure vessel).
Additional documents pertaining to vendor/subcontractor contracts, if relevant (e.g., commissary, security, facilities).
Policies and Procedures of the Facility
Rules of the facility and employee manual
Discipline, benefits and allowances, hiring and recruitment policies, child labor policies, employment conditions and wages, grievance procedures, freedom of association policies, health and safety policies, environmental policies, and training policies.
Administration of Employee Documents
Personnel files of employees (including but not limited to resignation records, labor contracts, copies of identification documents, employment applications, and disciplinary notices)
Health examination and employee registration records (if applicable)
Personnel files of employees (including but not limited to resignation records, labor contracts, copies of identification documents, employment applications, and disciplinary notices)
Attendance and Payroll Documents
Time records (regular and overtime), payroll records, and piece rate records for the previous 12 months (including the most recent period, low period, and government waiver/overtime permit) are all required.
Production and piece rate documentation (e.g., daily production report, product inspection report)
A compilation of recognized festivals
Records of earned and casual leave for the previous two years
Documentation of Social Security Contribution
Documentation pertaining to employee benefits, such as maternity leave and annual leave.
Documents pertaining to the Freedom of Association, including legal organization agreements and meeting records
Safety and health in the environment (EHS)
Plan for facility evacuation or layout
Manual and/or written EHS initiatives
Documents of the EHS Committee (agendas, minutes)
Documentation of EHS training (including personal protective equipment and chemical safety)
Security & Health
Reports on equipment and workplace safety permits, licenses, and testing
Service and inspection records for equipment
A compilation of the hazardous procedures and chemicals
SDSs are safety data papers.
Enumeration of mandatory personal protective equipment
Records of inspections, testing, and maintenance of fire suppression apparatus
Records of fire drills and fire inspections
Procedures for emergency action (including an Emergency Response plan)
Documentation of occupational illnesses and injuries
Documents documenting the surveillance of industrial hygiene (including chemical exposure, air contaminants, noise, temperature, lighting, and interior air quality).
